Hawkesbury Library Service - Conditions of Membership

On producing an acceptable form of identification at either the Central or Branch library, you will be issued with a library card which will need to be signed.

• You are responsible for the return of all loan items on time and in good condition.
• If any items that you borrow are lost or damaged beyond repair, you will be required to pay the replacement cost and a processing fee per item.
• If you have overdue items your membership and computer privileges will be suspended until overdue items are returned.
• You are responsible for all items borrowed on your card. Please report lost cards to library staff immediately. You will be held responsible for any items borrowed before the loss is reported.
• Notify staff of any change to your address, name, phone number or email address.
• Members under 16 years of age are required to have a parent/guardian complete their application. Proof of child’s and parent’s name and current address is required, along with proof of guardianship.
• Parents/guardians are responsible for their child's reading and Internet use, and the safe return of their loan items. The Library does not practise censorship of the collection or the Internet.
• Your membership details are confidential and will not be disclosed to other parties.

Online Only Membership

By applying for an Online Only Membership, i declare that:

• I am 16 years of age or older.
• The information i have provided in this application is true and correct.
• I understand that providing false, misleading or incomplete information may result in the suspension cancellation of my library membership and access to library services.
• I agree to comply with the library's membership terms and conditions and all relevant library policies, aligning with the library patron code of conduct.
